CEP Cards
If your CEP card expires, please remember to sign up for a training session on the USA Hockey website.  
 
Coaching Forms
If you are planning on coaching this year but just don't know which team, you can still start filling out your forms.  They can be sent to the following address or dropped off at Fogerty.  Please make sure they are complete.    SLPYHA, Attention: Traveling Director, PO Box 49812, Blaine, MN 55449    You'll need the following forms completed.
  • Adult Consent to Treat form  
  • Copy of CEP Card (front and back)
  • Screening Form
  • Coach Application
  • USA Hockey Registration Number
  • Coach Code of Conduct form
 
Tournaments
Tournaments will be handled the same way it was last year.  Let me know if you have your eye on a specific tournament so we can jump on them.  We will see what we can do.  

Teams can register for two local and one out-of-town tournament (60 miles from Fogerty Arena).  SLPYHA hosted tournaments, if offered at your level, are considered one of the local tournaments.  Teams are budgeted $2,000 ($2,500 for Bantams and U14) that can also be applied towards gate fees.

 

Any amount exceeding the team’s budget cannot exceed $75 per player, it can’t be over 60 miles from Fogerty Arena, it can’t conflict with D10 scheduling and you have to make sure you fall within the squirt/U10 limits for games and/or practice ratios.  Please refer to the handbook for this.

 

It is important to discuss with parents/guardians if they are able to contribute additional funds towards additional costs that may incur due to additional fees involved for tournament participation.

 

Any out of state tournaments have to be approved by D10.

 
 
Dryland Schedule

The schedulers are not longer able to schedule dryland for you.  The process has changed.  You will need to contact Fogerty directly (fogertyarena@fogertyarena.com or 763-780-3328 Ext. 6).  Mite teams must let me know what dryland hours they have scheduled because they count toward the 3/1 practice/game ratio
