Spring Lake Park Youth Hockey Registration
First Year Skaters:
First Year Skaters at the 6U, Mite D, and Mite C levels will include a registration fee of $75.00 and Season Fees totaling $100.00. Two equal payments of $50.00 will be due on November 15th and January 15th.

Welcome to the 2011 - 2012 Spring Lake Park Youth Hockey Registration
We not only have a fantastic new website, but registration will be done online! To register for the 2011-2012 Season, please login or create a user name and password to begin the registration process.

You will be completing two separate transactions during the process. First you will need to register with USA Hockey to obtain your USA Hockey registration code. Our registration process will prompt you to this link. Once this has been completed, you will continue with SLPYHA registration.
You will be required to enter the USA Hockey registration code when prompted. In order to avoid multiple transactions, it is recommended that you register all your skaters with USA Hockey before continuing with SLPYHA online registration.
The online registration process will allow you to register all of your family’s players and pay via Visa, MasterCard, Discover or PayPal. There is also an off-line payment option if you choose.**
All players must be completely registered in order to be able to participate in pre-skates or start the hockey season. The following documents are required to be turned in and registration fees must be paid to be considered completely registered.

o Copy of Government Issued Birth Certificate
o Volunteer Deposit Check - $250 per child made out to SLPYHA
(this check is a deposit and will not be cashed unless volunteer requirements are not met)
We will be at Fogerty Arena on the following dates to collect paperwork.
· August 24th 5:30 pm - 7:00 pm
· September 12th 6:00 pm - 8:00 pm (Traveling Registration Closes)
· October 24th 6:00 pm – 8:00 pm (Mites, U6 and U8 Registration Closes)
If you are unable to attend any of the above dates, your required forms must be dropped off at Fogerty Arena at the SLPYHA drop-box (located on the door next to the Board room in the South rink) or mailed to:

These forms must be in our possession no later than:
September 12th for Traveling players
October 24th for Mites, U6 and U8 players
PLEASE: Do not give any forms to Board Members, Coaches or Managers directly.

Past Due Accounts: Players with outstanding balances from the prior season will not be allowed on the ice

**Home Ice includes Practices and Home Games. These hours do NOT include Tournaments, Away Games, or Dryland.**
**There may be a plus or minus factor of %5 on number of hours received due to ice availability**

SLPYHA is happy to announce a partnership with ProHybrid to provide Top Quality Goalie Training for all Traveling Goalies. See below for more information:
On Ice Goalie Clinics:
- 8 sessions throughout the season
- 1.5 hours per session
- Estimated 15-20 goalies per session
- 5 ProHybrid Instructors per session
Off Ice Goalie Clinics:
- 4 sessions through season (OCT, NOV, DEC, JAN)
- 45 min Consistent Confidence Seminar
- 45 mins Goalie Specific Dryland
- Each goalie will get a personal workout guide to follow between the monthly clinics
