Volunteer Requirements have been simplified

Additional Ice Bills will be assessed, amounts to be determined soon

To be eligible to play hockey for SLPYHA you must abide by MN Hockey guidelines and reside within the boundaries of ISD#16. Open Enrollment to ISD#16 does not change residency rules per MN Hockey.

SLPYHA Members with past due accounts will not be able to register until full payment on their balance has been made. If financial assistance is needed - contact the Treasurer, see the Board page for contact info

In-Person Registration Dates:

Saturday - September 6th - 1:00 - 4:00 PM - Fogerty Arena

Thursday - September 11th - 7:00 - 9:00 PM - Fogerty Arena

Mail in Registration
Must be received by August 27th

STEPS TO REGISTER:

1. Register Online with USA Hockey – NEW THIS YEAR

(players only, coaches register with SLPYHA)

2. Complete the following forms:

➢ SLPYHA Registration Form

➢ Consent to Treat Medical Form

➢ You must bring/mail the confirmation page from USA Hockey

3. Write checks for:

➢    Registration (See form for amount)
➢    Equipment/Traveling Jersey Deposit $250
Separate check, returned if equipment returned in good condition by April 1st
➢    Volunteer Deposit $250
Separate check, returned if hours completed by April 1st
➢    Fundraising Candy $52
You may opt out for $33, taking no candy

4. Register by Mail - Must be received by August 27
